The Progressive Approach to Home (PATH)

The Progressive Approach to Home (PATH) is a unique, transitional care model that utilizes evidence-based practices and medical models. The facility uses this program and transparent communication while working to return patients to their homes as quickly and safely as possible and reducing their number of hospital visits.
